We believe that everyone, regardless of their financial means, deserves to write their own final chapter in the way that’s right for them. To that end, we offer financial assistance for those in need of support that’s not available through the insurance model, or is difficult to find and fund.
Our Support
Reducing financial barriers and Increasing access to care
If you have concerns about the financial burden associated with your end of life experience or someone else’s, contact our team to apply for financial assistance. Our nonprofit provides assistance on a case-by-case basis, based on guidelines for access and our capacity to provide it. Qualified applicants may be waitlisted due to limited resources.

How to Apply
Step 1:
Complete Short Application
Currently available by request. Contact us to request the application. Applicants will provide contact info, a financial overview, physician and pharmacy info, and desired service/support.
Step 2:
Verification & Review
Our team will review within 5–7 business days. A staff member may follow up with applicant, Doctor, or pharmacy for clarification or supporting information.
Step 3:
Notification & Disbursement
Eligible applicants will receive a response via email or phone. If approved funds will be paid directly to providers or vendors. In some cases, prepaid service vouchers may be issued.

Currently available by request. Contact us to request access to the application. Applicants will provide:
- Basic personal and contact information
- Financial overview (income, expenses, insurance status)
- Care team and pharmacy information
- Desired service or type of support requested:
- Medical Aid in Dying (MAiD) – physicians, medications, ingestion support
- Death Doula Support – emotional, spiritual and practical guidance
- Bereavement Support – free support groups and access to 1:1 care
- Bedside Nursing Care – bring peace of mind with expert clinical support
- Urgent End of Life Housing – go from hospital to “home” for your final days
- Voluntary Stopping Eating and Drinking – legal option care and counseling
- Other – please describe.
- The Foundation team will review within 5–7 business days. A staff member may follow up with applicant, Doctor, or pharmacy for clarification or supporting information.
- Once Doctor/Pharmacy approves support, we applicant of approval and that the Dr. or Pharmacy will be in touch with next steps. We will work directly with the Dr. or Pharmacy to make necessary payments.
Eligible applicants will receive a response via email or phone if approved within 5-10 days.
Funds will be paid directly to providers, pharmacy, or vendors. In some cases, prepaid service vouchers may be issued instead.
